Adding nodata to the meta dict exposed a problem where that
property would be left undefined when a dataset was closed.
Also document the new context manager in the README.

Change band indexes to start at 1.
...
read_band(1) gets the first band (formerly index 0). New indexes attribute
returns a list of band indexes. A 3-band raster will have indexes: [1,2,3].
Fixes #2 .
