Fixed misleading documentation

options.xfwd definitely works fine without using the .listen method, and, AFAICT, .toProxy should as well. Only .ssl and .ws are referenced in .listen.

Firefox sends `keep-alive, upgrade`, not just `upgrade`,
which the proxy incorrectly turned into `close`
